Cardiac Rehabilitation



Madonna ProActive’s Cardiac Rehabilitation Program, an outpatient service of Madonna Rehabilitation Hospitals, was developed using a holistic model, combining all aspects of a person's wellbeing including social, emotional, spiritual and intellectual health, as well as physical health.

Individuals will receive medical expertise in a supportive environment provided by a team who will work together to help them succeed.

It was designed to help heart surgery patients recover or possibly prevent the need for heart surgery for others. Starting with an individualized health assessment conducted by a cardiac nurse, our expert team works with participants to establish an individualized program to help them meet their fitness and health-related goals.

Because we believe you are an integral part of the goal-setting process, you are an active member of your Cardiac Rehabilitation team, which includes a cardiac nurse, exercise physiologist, health psychologist, registered dietitian and relaxation specialist.

To meet your goals, we will use approaches found in traditional cardiac programs such as monitored exercise, progressive activity and education. We will complement these traditional approaches with unique techniques including cardiac or chair-based yoga, guided imagery, relaxation techniques, massage, Tai Chi and heart-healthy cooking. We encourage you to select topics from each of our major educational categories that interest you: cardiac risk factor modification (to include smoking cessation), nutritional counseling, prescribed exercise and holistic health counseling.

AACVPR Certification

American Association of Cardiovasular and Pulmonary RehabilitationThe Cardiac Rehabilitation program at Madonna ProActive has met the strict standards set by the American Association of Cardiovascular and Pulmonary Rehabilitation to promote health and prevent disease through AACVPR Program Certification.

Program Certification and Recertification designation means the facility meets the essential standards of care described in the 4th Edition Guidelines for Cardiac Rehabilitation and Secondary Prevention Programs. Each program is reviewed by the AACVPR National Certification Committee and the AACVPR Board of Directors for final approval.

The mission of AACVPR is to reduce morbidity, mortality and disability from cardiovascular and pulmonary diseases through education, preventions, rehabilitation, research and aggressive disease management, through its integrative medicine programs and facility memberships.

The cardiac team exercise physiologist will provide you with a step-by-step introduction to ProActive's exercise equipment. You will also be taught how to monitor your exercise during your workout and follow through on your program. You will begin Cardiac Rehabilitation under close supervision of the staff using heart monitors. This supervision will decrease as your health improves and you gain the knowledge, understanding and confidence to advance your exercise. Our goal is to help you become an independent member of ProActive within three to five months. ProActive offers a variety of exercise equipment for all fitness levels.

The real difference in our program is the integration of traditional and holistic approaches to a program that is individualized just for you. We recognize you as the driving force in choosing key areas that will impact your health.

Because we limit class size, you will have optimal personal attention to address your individual needs. This ratio is ideal for both your monitored exercise, as well as your risk factor education. We understand that accepting your diagnosis and making lifestyle changes isn't easy. We will work one-on-one with you to set goals and provide the support you need to stay on track.

It is a Medicare-approved program that covers a variety of diagnoses including myocardial infarction (heart attack), open-heart bypass surgery, heart valve surgery and coronary stent placement. The program will be covered by most private health insurance plans with a physician referral.

    Holistic Health and Integrative Medicine Coordinator, Cardiac Rehabilitation

    Sharon Duffy, RN, MS, CRRN, has been a nurse for more than 40 years practicing  in a wide variety of clinical and community positions. She began studying in the wellness field in the early 1980s and received her master’s degree in Wellness and the Family from the University of Nebraska-Lincoln. She has been a grief counselor for families experiencing the death of a child for 25 years. She now serves as the Wellness and Holistic Nurse at Madonna ProActive.

    Registered Dietitian, Personal Trainer

    Lisa Graff, MS, RD, LMNT, ACSM-CPT is a registered dietitian and licensed medical nutrition therapist, as well as a certified personal trainer. She has experience in the areas of weight loss and weight management, pregnancy and lactation nutrition, cardiovascular health, diabetes management, food allergies and intolerances and more!

    Cardiac Rehabilitation

    Sheryl, a Minnesota native, graduated from Augustana College in Sioux Falls, South Dakota with a bachelor's degree in Nursing.  She later continued her education at Andrews University in Berrien Springs, Michigan, receiving her master's degree in Nursing and became a Clinical Nurse Specialist.

    She has been a Cardiac Rehab Nurse since 1985.  She has been an active part of the Cardiac Rehabilitation Team at Madonna ProActive since August 2006.  She is also trained to be a foot care specialist and is near completion of her certification of becoming a Certified Diabetes Educator.

    Dr. Jerry Reed, a Nebraska native, has been an active member of the ProActive Cardiac Rehabilitation team since 2007.  He currently serves in the capacity of Medical Director .  He is present during every session.  He received his medical degree from the University of Nebraska Medical College and has been an active practicing physician in Nebraska continuously since his graduation. He has served the community as an Internal Medicine Practioner for many years.

    Cardiac and Wellness Nurse

    Monica is a rural Nebraska native, former Army Reserve Nurse, who earned her nursing diploma from Bryan Memorial Hospital School of Nursing, attended Doane College, and earned her bachelor's degree in Nursing from Nebraska Wesleyan University.  She has cared for cardiac patients in a variety of settings, including in-patient Telemetry, Cardiac catheterization and electrophysiology labs and out-patient Cardiac Rehab.  Monica maintains her BLS and ACLS certifications.  She enjoys working with a variety of people helping them live a healthy lifestyle by guiding them to take an active role in achieving their goals.   In her spare time she enjoys volunteering at The Gathering Place, Matt Talbot Kitchen, short-term mission trips to Honduras, and in various study groups and Sunday school curriculum committee at her church.  In the past, she has volunteered for Clinic With a Heart.
